All patients are advised to provide a sperm sample after 3 and 4 months to ensure that the operation was successful.
Two sample containers and lab requisitions are provided to each patient. There is a $25 fee for providing additional containers or requisitions by mail when the originals are lost or misplaced. This can also be provided at no charge in person at the office during regular office hours.
Patients who purchase the Premium Package will be able to view their results online.
Those who do not should schedule a follow up appointment after each test to review their results. Results can also be viewed online ($50).
